Patient Financial Responsibility
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will file your claim to the insurance company for you. At the time of service your deductible and estimated percent out-of-pocket will be due. We would be happy to discuss payment arrangements if needed prior to the actual procedure. Any balance due after the insurance company has paid is your responsibility. Most insurance companies will respond within four to six weeks. Following this, we will send you a monthly statement.
